FSO Electronic Forms Distribution Option
Advantages:
  • Distributes company forms via the World Wide Web.
  • Field accesses and prints forms on demand from the company's private area within FSO or from the company's private Web site.
  • Provides a central source of current forms.
  • Easy to manage and update.
  • Would eliminate some or all of the current home office forms-related paper, printing and shipping costs, depending on the degree of implementation.
 
FSO Services:
  • Development of state-specific forms menus.
  • Conversion of forms to the Portable Document Format (PDF) and placing on Web server.
  • Linking of forms into state-specific menus.
  • Testing of state-specific forms delivery.
  • Replacing forms with updates/new editions.
 
Pricing (effective 2/1/96):

Implementation Fee per State-Specific Menu (1) :

  • Initial Implementation: $50 plus $5 per form on menu
  • Later Additions: $20 per form per menu
Annual Maintenance Fee (2) : 20% of Implementation Fee
(1) Assumes forms are provided in a convertible software format (e.g., Word6, PageMaker, Quark)
(2) Includes replacing forms with updates/new editions and testing.
 
Example:
50 state-specific menus with an average of 25 forms per menu
Initial Implementation Fee:
$50 + ($5 x 25 forms) = $ 175 per state
x 50 states
$8,750 total
 
Annual Maintenance Fee:
$8,750 x 20% = $1,750
